the order summary. If everything is correct, click on the

"Ordering Method" list and click on the

the summary of your billing and shipping information. If
everything is correct, click on the "Correct"

then have one final opportunity to review and modify your
order. If everything is okay, click on the "Prepare Order
Form" button and a "printer-friendly" fax order
form will be displayed. |

this form and write in your credit card information and sign the form in the spaces provided. Then fax it to us
